- You may wish to use these forms.
- There are a number of models/workflows that people use to enter information.
- Gather and enter proficiency scales first using the “Enter by Subject” feature and then compose the written portion using the proficiency scales as a reference.
- It is advisable to compose the written portion in a word processing application (Word, Pages, TextEdit) and save the document(s) in Rich Text Format (RTF) before copying and pasting them into the comments field in SSDAS.
- Alternatively, you can fill in information individually by student.
- Whichever way you use, it is advisable to keep a record of what you input.

- Gather and enter proficiency scales first using the “Enter by Subject” feature and then compose the written portion using the proficiency scales as a reference.
- All grade 4-7 students will need to have a letter grade inputted for all areas of study that will transfer to the Permanent Student Record. This is still a requirement according to the Ministerial Reporting Order. It will not show on Form B summaries. See memo sent (or will be sent) to your administrator for details.
- Proofread your reports to make sure that the information is correct including placement of arrows on Form B summaries.
- Make sure you have included a statement indicating the student has been assigned to the next grade.
- Inform your clerical/principal that your reports are ready for printing.
- Do a final proofread of the printed report before signing
